Business Account Requirements
Below is a list of documentation that is required, depending upon the type of account, when opening a business account at Meriwest Credit Union:
ALL Business Accounts MUST provide the following documents:
- Business Account Application Agreement and Signature Card (available upon request)
- ATM/Debit Card Request Form (if the business wants to order an ATM/Debit Card)
Sole Proprietorships
- Fictitious Business Name Statement (FBNS) (if applicable)
- Online Verification of FBNS (if applicable)
- Beneficiary Designation Form (If owner chooses to designate)
- Identification on ALL signers – 2 forms of ID (primary and secondary)
General Partnership
- Partnership Agreement OR Letter of Authority (showing all officers and their authority)
- Online Verification of FBNS (if applicable)
- Identification on ALL signers – 2 forms of ID (primary and secondary)
Foreign GP:
- All of the above, PLUS
- Statement by Foreign Partnership and Designation (LL-27)
Joint Venture
- Joint Venture Agreement
- Fictitious Business Name Statement (FBNS) (if applicable)
- Online Verification of FBNS (if applicable)
- Identification on ALL signers – 2 forms of ID (primary and secondary)
Limited Liability Company (LLC)
- Articles of Organization (LLC-1), Plus any Amendments (LLC-2)
- Online verification that LLC is still active
- Operating Agreement
- Fictitious Business Name Statement (FBNS) (if applicable)
- Online Verification of FBNS (if applicable)
- Identification on ALL signers – 2 forms of ID (primary and secondary)
Foreign GP:
- All of the above, PLUS
- Limited Liability Company Application for Registration (LLC-5)
Limited Liability Company (LLC)
- Certificate of Limited Partnership (LP-1)
OR Limited Partnership Agreement (showing all officers and their authority) - Fictitious Business Name Statement (FBNS) (if applicable)
- Operating Agreement
- Fictitious Business Name Statement (FBNS) (if applicable)
- Online Verification of FBNS (if applicable)

Limited Partnership

- Online Verification of FBNS (if applicable)
- Identification on ALL signers – 2 forms of ID (primary and secondary)
Foreign GP:
- All of the above, PLUS
- Statement and Designation by Foreign Corporation
- Certificate of Qualification (with the State of California)
Unincorporated Association
- Letter of Authority OR Board Minutes
- Identification on ALL signers – 2 forms of ID (primary and secondary)

OR Application to Register (LLP-1)

Limited Liability Partnership
