Proxy Authorization


A Proxy is a written authorization signed by a member giving a designated person or persons the power to vote on the member's behalf. You are not required to sign a Voting Proxy as a condition of membership with Meriwest; rather, signing a proxy is an option that is available to you to allow another person to exercise your voting rights when you are not present in person to do so yourself. Having a Proxy means your vote will be counted, even if you can’t attend the Annual Meeting.

If you would like to re-appoint the Board of Directors (or designee) of Meriwest Credit Union as Proxy Holder to exercise your voting privileges, please complete and submit a new Proxy. Upon submission, this will constitute your signature on the proxy and will authorize the Proxy Holder to vote on your behalf.

Your new Proxy shall be valid for a three-year (3-Year) term commencing from the date of execution or electronic submission.

If you choose to authorize a Proxy to vote for you, please be advised that your Proxy may be voted by the Proxy Holder as outlined in the Proxy Form below.

I affirmatively consent to receive communications and disclosures electronically to proceed with this online form. I have read and agreed to the terms in the Agreement to Receive Disclosures Electronically.


Proxy From

I hereby appoint the Meriwest Credit Union Board of Directors, by majority vote, to appoint a proxy as my attorney in fact and to represent me at all meetings of the members of this Credit Union, to vote for me in my name on all questions and elections coming before said meetings, to give consents and in all other ways to act for me at said meetings. The proxy shall remain in full force for three (3) years from the date set forth below, unless revoked by me by a subsequent proxy, or otherwise in writing.

BY CLICKING ON THE SUBMIT BUTTON BELOW, I HEREBY REVOKE ALL PREVIOUS PROXIES, AND I RESERVE THE RIGHT TO WITHDRAW THIS PROXY FROM ANY MEETING I ATTEND AND VOTE AT IN PERSON.

I hereby authorize my proxy to vote for me in my name on all matters, including all of the categories set forth below:

  1. For the election of directors and members of the supervisory committee of the Credit Union to serve for a term of three (3) years and until their respective successors shall be elected and shall qualify.
  2. To vote on the adoption of Amendments of the Articles of Incorporation and Amendments to the Bylaws of the Credit Union, except amendments repealing, restricting, creating or expanding proxy rights.
  3. To vote on the principal terms of any merger between the Credit Union and any other Credit Union or other business entity.
  4. To vote on such other proper business as may come before such meetings.

IMPORTANT NOTICE – PROXY USE If you have appointed the Credit Union’s Board to act as your representative by means of a current, unrevoked proxy, the proxy will be voted by the Board’s authorized designee at member meetings on director and supervisory committee elections, amendments to the articles of incorporation and bylaws, principal terms of any merger, as well as such other proper business as may come before such meetings. Your proxy may be revoked by a subsequent proxy, or otherwise in writing. You may also withdraw your proxy from any meeting you attend in person.
